| I have seen a lot of injuries and have had quite a few myself since training BJJ. Most common big injury is usually ACL tear which my girlfriend just did and had surgery for last month. I would say the bigger injuries are usually a fluke thing. Like TRIANGLEFROMGUARD was describing, it's not usually from not tapping to a submission it's usually something weird...an arm in the wrong place, foot stuck in the mat when you try to pivot...sprawl hard on a guy's shoulder...etc. People who refuse to tap and break something is their own fault, that's different.
