Yep, it's always something weird like that. My injuries have been:

Herniated disks in my lower back: I tried to push my hips up for an armbar when I was stacked. Stupid.

Broken toes: All in all I've broken 6 toes. Usually this is from my toes sticking in the mat as I pivot.

Severely spraigned ankle: Not tapping to toe holds. Stupid.

Torn meniscus: Pushing my leg in half guard at the same time someone was pushing my knee down. Stupid.
