| Matt, hang in there. I know it's not easy to lose weight when you don't know what to do. You can get pretty decent advise from here but you may really want to get some professional help even if it cost some money. Your health is definitely worth it. Find the personal trainer who will work with you, both in the gym and give you good menu.
Few tips. Stay away from fast food. Stay away from carbonated drinks. Drink pure water and fruit juice. If you have to eat out, try like Jamba Juice smoothies instead of McDonalds.

These are just different options. Feel free to mix it up so you don't get bored.
Breakfast: Cereal with skim milk (or even soy milk), yogurt, banana, apple (or other fruit)
Lunch: chicken breast sandwich, tuna sandwich (light on mayo) on whole wheat, turkey sandwich, Protein shake, chicken salad
Dinner: chicken breast (not fried), salmon, salad, tofu, fruit,
